E/M and Emergency Visit on same day

First scenario: patient is coming to the urgent care center and the physician already knows the patient will be transferred to the emergency room after vitals and EKG.

Second scenario: patient comes to the urgent care center and the physician performs vitals and EKG then determines the patient should be transferred to the emergency room.

How should the urgent care physician bill for his services if only one E/M code can be billed per day?
