For CPT® codes 33254-33259? I don't see any ncd for these codes. I have billed them for years without issue. Do you have a specific question about those codes? I can try to help.

Thank you for responding. I should have listed the codes. We are not doing mazes we are using the codes 93656 with 93613 and 93662 with 26 mod. I have a local insurance denying for the dx of I48.1 which they are stating is not in the NCD from medicare that they follow.

Where do I get a copy of the NDC so I can use it for the appeal?

I use Encoder Pro and it's on that program. I can not find a policy that shows acceptable dxs on the internet. I can fax or email you the pages I have printed from there that show acceptable dxs for CPT® code.
