No modifier, the provider is identified by their UPIN number. This cannot be an "incident-to" encounter because physician is not in office, The NPP bills under their own number.
The only modifier used to identify an NPP as assist at surgery is AS

There are two articles in the database about incident to services, and one on shared services.

Search for incident to in the "look it up" box for a complete explanation.

If the doctor is not in the office, you may not bill under the doctor's provider number for a PA. Bill under their own provider number and be reimbursed at 85%.
