CPT® code 36595 removal of pericatheter and insert a replacement catheter

Good Afternoon,

On the same day, the physician will remove a mechanical pericatheter CPT® code 36595 & 75901, and then perform a replacement tunneled central venous catheter CPT® code 36581 & 77001.

The payers will pay for the replacement but not the removal. I'm missing a modifier, or is the removal on the same day, a non-covered service?
