Medications: Adipex - ordering provider Dr.xx

The above is a nurse visit for the refill of Adipex, Nurse is refillthe Adipex under the supervision of Dr.xx. In the superbill Nurse reported the CPT® as 99212, as per my knowledge as the nurse is refill only medications it should go with 99211. Please clarify whether it should go with CPT® 99211 or 99212

Why does a medication refill require a nurse visit in the first place??

But, if medically necessary and documented, a nurse may not report a service higher than 99211.
