regarding chemo administration

what are suggested CPT® for chemo administration in office ? because when we use CPT® 99360 along with CPT® 99213 it doesnot work. we need to know hierachy of codes .

Chemo is 96413 for the initial drug 1st hour. Chemo IV is listed first as the initial drug, Theraupetic, Prophylactic and Diagnostic IV and Injections (96366-96376) listed second with the subsequent codes and last is the Hydration codes (96361) listed as subsequent. Remember you can have only one initial administration code per session. If the Hydration is the only thing that is being done then 96360 would be the code.

If an office visit is being billed also, then Modifier 25 would be on the visit.
