Repeated Denial of 99202?

Hi there!

I work for a cardiologist and am having the toughest time with one particular code.

Billing the codes 93350, 93018 and 99202 together, some companies will deny 99202 as bundling. Medicare will pay it, but most others won't. This is a recent change, as I've been billing this combination for some time with no issues.

Is there a modifier I can use to help with this? I tried 22 with no luck.

Will also deny it when I bill it as part of a Lexiscan/Nuclear Perfusion test with the codes 78454.26, 93018, and 93016.

Any help would be great. Thanks!

are you putting a modifier on the E/M??? if you're trying to bill it the same date as a diagnostic test then it needs a -25 modifier to separate it from the test.
