Body areas and organ systems

I have a question regarding body areas and organ systems using the 1995 guidelines.

When reviewing the physical exam how do I determine enough has been documented to count a body area or an organ system? I get confused mainly on the organ systems.....I don't want to give a provider credit for the organ system if there isn't enough meat......is there any sort of checklist to go by?
