New codes for injectable testosterone and urine drug screen

I have been using J1080 for testosterone cypionate 200mg/cc but this code was retired on 12/31/14. New code from what I can tell is J1071 but this code is rejected at my clearinghouse.

I have been using 80104 with mod QW for commercial insurance urine drug screen but again, it seems this code was retired. I thought the new code was 80300 with mod QW but this is being rejected at clearinghouse as well. Any help with either of these? I am stumped.
