New patient level 3 HPI

If patient presents with 3 different complaints do I need 4+descriptors for each complaint or a total of 4.

You don't need it in order to justify the detailed or comprehensive history. You need only 4 total elements of hpi in the hpi. That said, you want a well written note to capture a good history for each condition. So I wouldn't necessarily get to 4 total then stop.
