CPT 27446 and CPT 27437 Billing Question

Hi - I am having an increasingly more difficult time getting my unicompartmental knee (27446) and patelloplasty (27437) codes paid. I have been appending Mod. 59 on the patelloplasty but it is still being denied. Is there something new to indicate that this non-payable? thanks.
