How do you bill CPT® codes 99203/92285/92071?

1. Modifier 25 required for same day E/M service
2. For private payers, consider modifier 51 with secondary and onwards code(s)
3. 99285: A separate report and interpretation needs to be reported. It should document which eye is photographed, date taken and reviewed, and abnormalities labeled. The camera should be of a special type capable of taking magnified images. Appropriate diagnoses codes for medical necessity are required, as photos taken merely to enhance the patient's chart are not allowed. Photos may be of eyelids, lashes, cornea, conjunctiva, sclera, iris, lens, and the angle between cornea and iris.
4. 99285 & 92071: Bilateral modifier 50 might be required based on documentation
