Do you bill labs at time of collection or at time of result when sending to outside laboratory

Hi-

We are setting up an EHR charge interface and are having heated discussions regarding whether or not we bill at the time of collection or at the time of result.

Would love to hear everyone's experiences.

I have always billed at time of service, (i.e. venipuncture, urinalysis), but lately AR Medicaid have been denying my claims " as incidentals". Do I now need to append a modifier to the E/M? I have called Medicaid with no answer yet.
