Social Adult Day Care and MLTC billing

How to bill Adult day care? What claim form should we use, UB04 or CMS1500? If UB04 which Occurrence and value codes should we use?, type of bill? How to bill non emergency transportation to the Adult day care?

Every MLTC company has their own requirements.
You should contact a provider relation representative for every MLTC company that you have a contract with. They will let you know what is the proper way to bill them and what form to use. UB04, CMS and electronic billing are all used by different companies.
