Carve outs for Medicare

Can someone give me some information in regards to how to bill for carve outs? Can any covered service by Medicare have a carve out? Need more specific guidlines! We are a multi physician practice group and do everything from surgery to office visits. Any suggestions are greatly appreciated!!

can you give an example of what you mean by a "carve out" on a Medicare patient?
