BCBS Denial of 80104

Just had BCBS out of state plan deny 80104 stating that it is the incorrect procedure code to use. Should this code have a QW modifier for it to pay? The remark code just said incorrect procedure code - not incorrect modifier, so not sure what to do.

You will need to reference the medical payment policy for the state processing the claim. I am in Georgia and this is their stance:
Drug Screening Services: BCBSGa has a new reimbursement policy, Drug Screening Services, regarding the reporting of Qualitative Drug Screen Testing, which will be effective with claims processed on or after January 1, 2013. We have adopted the Centers for Medicare & Medicaid Services guidelines and will no longer reimburse CPT© codes 80100, 80101, and 80104. Qualitative drug screening will now only be reimbursable using codes G0431 and G0434. Both codes G0431 and G0434 will be eligible for 1 unit of reimbursement per date of service. Please refer to the full text of the new Drug Screening Services reimbursement policy for additional information. CPT© codes 80100, 80101, and 80104 will also be added to our Bundled Services and Supplies reimbursement policy.
