Place of service denial.. Please help!!

Hi Everyone,
We billed out 99243 and 21501 with POS 11. Phoenix Health Plan denied for wrong place of service. YES 21501 is a BIOPSY but he performed in the office. We have documention. Am I missing something?? By the way TOS for 99243 was a 3. TOS for 21501 was 2. Can anyone please help? Thanks Stephanie

Sorry, you should try looking into the CPT® book, or taking a crash course or getting someone in there to help you out a bit, POS codes, its the first page of CPT

I respectfully agree with that you MUST go back to your CPT® book under E/M review your guidelines. In the interim, I can email you a crosswalk as Medicare no longer accepts consult codes as of Jan 1, 2010 the crosswalk will aid you in selecting the correct code but it is by no means a shortcut to not knowing the guidelines. Also, review your POS codes check this link out for those http://www.cms.gov/Medicare/Coding/place-of-service-codes/Place_of_Service_Code_Set.html. You can do it....BEST WISHES! Weird, because both can be done in office. MCR like payers do not cover consult codes but that would be a different denial.
