Need help for denial of CPT® 91110

CPT® 91110 denied by GHI Medicare (New York, Queens city) as "treatment was deemed by the payer to have been rendered in an inappropriate or invalid place of service". Claim was billed with POS 22 (Hospital outpatient). Please help. Thank you!

I bill the professional portion only at outpatient hospital and get paid by Medicare. I live in Tennessee.
