Path charge (88305) same day as Mohs surgery, unrelated to Mohs surgery.

Does anyone know how to get a path charge that is billed on the same date as Mohs surgery, but is an unrelated site to the Mohs surgery, paid by Medicare? They will pay for the biopsy, but I can't find any modifier or combination of modifiers that will get the pathology charge paid. Again, this is an unrelated lesion that is biopsied (not a frozen section biopsy) on the same date as a Mohs surgery is performed.

I have tried 59, but it does not work. We bill for the professional component with modifier 26, have tried 26-59 too, to no avail. I guess I just have to tell the doc to biopsy at a post op visit instead of during Mohs surgery!
