77003 fluoroscopy

I am being denied for 77003 being bundled with 64460. The cci edicts do not say that it needs a modifier, and cpt book says it is not bundled.

Any one else having a problem with these two coded being bundled? And what was the result?

Fluoroscopy

there is no 64460 CPT code.
It is important to note that there are general restrictions on the use of fluoroscopy codes. The American College of Radiology (ACR) points out “a general rule of thumb is if fluoroscopy is always performed as a part of the radiological imaging study, fluoroscopy is included in the radiological procedure code.” In these cases, fluoroscopy should not be coded or reported separately.
Code 77003 (Fluoroscopic guidance and localization of needle or catheter tip for spine or paraspinous diagnostic or therapeutic injection procedures [epidural, transforaminal epidural, subarachnoid, paravertebral facet joint, paravertebral facet joint nerve, or sacroiliac joint], including neurolytic agent destruction) is used when guidance is required for spinal or paraspinal injections to direct or localize the needle or catheter tip. Therefore when guidance is provided in conjunction with codes 64470-64476 and 64479-64484, code 77003 is reported since these codes are used for spinal procedures. Code 77003 should not be reported separately when a diagnostic epidurogram is provided.
