Supporting Documentation for CPT® 96110

Hello...I'm looking for references that address whether or not the COMPLETED FORM for developmental testing such as PEDS & M-CHAT must be retained as a part of the medical record when there is documentation of the interpretation/results (based on the replies on the form).

Are there any sources which address this issue that you can recommend? Or is there anyone you know of who might be able to assist me?
