Pregnancy complications

I was wondering how many coders code anything that happens during the pregnancy as a complication to the pregnancy whether the clinician states as such or not. For instance, pregnancy in a smoking mom. Anyone code this as a complication to the pregnancy? How about a complication on the newborn chart? Thanks for your comments!!
