DMV Physicals

We recently had one of our physicians become certified to do DMV physicals. Is CPT® code 99455 the appropriate CPT® code to use to submit charges? I know this code is used for Workers Comp but I need to know what would be appropriate CPT® code to use.
I was also told V70.3 would be the appropriate diagnosis code as it includes driving license. Please advise. Thanks!
