L57.0 benign keratosis
I am billing for an excision of benign lesion. The path report came back showing "benign keratosis". The ICD-10 code for benign keratosis is L57.0 which is the same code for actinic keratosis (Also known as pre-malignant lesion).
If I bill the 11400 excision of benign lesion, will Medicare deny the claim with L57.0 since it actually reads actinic keratosis?
