Medicare secondary paper claims

I fcannot seem to get the fields right on the Medicare secondary claims.

i am a family practice with 3 doctors. Not set up for claims to go electronically at this point.

could someone send me a completed Medicare CMS-1500 to show what goes on what line for the npi's and tax id's and such.

i would greatly appreciate it.
