benign lesion

We are trying to get Medicare to cover a benign lesion 3cm 706.3
excision. They do not like the CPT® 11403. Does anyone have any suggestions? The lesion is on her back and does interfere with her daily living. Thanks!!!

Well it looks like we are using the wrong ICD. should be 702.11. Thanks
