Could the lab results be used for Severity; say "size of the tumor 1.5, early stage, low/high grade, Gleason score"?

How about in a patient with breast cancer can the ER+ PR+ and HER-2 be used as Quality?

Diagnosis: Squamous cell carcinoma of the right retromolar trigone.
?Stage: Clinical T4a, N2c, IVb.
?Performance Status: 0/1
?Pain Scale: 0

DOS: 12/17/14
Diagnosis: Squamous cell carcinoma of the right retromolar trigone.
?Stage: Clinical T4a, N2c, IVb.
?Performance Status: 0/1
?Pain Scale: 0

Should credit be given in both visits for:
Location - retromolar trigone
Quality - squamous cell carcinoma
Severity - T4a, N2c, IVb or performance status 0/1

Can the pain scale be used in the ROS as constitutional?
