iud Mirena

Does anyone know how to bill Noridain Medicare for IUD Mirena for a patient with endometrial hyperplasia? The J code J7302 is not covered and we billed the unlisted code 58999 with notes in our electronic claim with the information that the patient can't tolerate oral progesterones.
Thank you for any help.

not sure this is any help, or only more confusion:

does it have anything to do with the differences btwn the bio-identical/faithful progesterone (compounded), . . vs. . . drug-company-manipulated/machined progestins?

Why are you using the unlisted procedure code 58999, that may be why MCR is denying it, they do not pay on unlisted procedure codes. Try using 58300 for the insertion of the device w/the J code and send it w/the office notes for medical necessity.
