Can Somebody Please Provide a List of All the Physical Therapy Codes for Medicare and Commercial Insurances?

Billing Medicare for PT is much more complicated than just billing physical therapy codes, you must also be aware of the functional limitation reporting guidelines and how to code them. Also bundling rules are complicated for PT so be aware of when it is appropriate to use modifiers and when it's not appropriate to use them only for avoiding bundling.
The only PT codes we usually bill are 97001/97002, 97140, 97530, 97110, 97035, 97012, and 97014/G0283

Medicare follows all of the edits from the National Correct Coding Initiative (Google it, the correct website is the first result). The NCCI regularly releases lists of CPT® pairs that will be bundled automatically.
For Medicare we append modifier 59 to every code. This doesn't prevent the NCCI bundling but it will unbundle any CPT's that are eligible.
