medicare denied 64450

Medicare denied charge 64450 with dx codes 723.1 and 355.9 as non covered services because this is not deemed a medical necessity. Can anyone help?

Any time you receive a denial for medical necessity you should be able to get the Local Coverage Determination from your intermediary. They usually have this on their website. I checked this and our FI does not have a LCD for the CPT® 64450. Sorry I'm unable to help more. Who is you Medicare intermediary?
