S-codes

I am a physician looking to bill a S9500 for providing
Drugs,supplies and nursing care to a pt going home w iv infusion therapy.
Is there any specific way this needs to be billed and charted if we have auth to use the Scode?

What happens when a home health company is also seeing the pt to help w wound care and help w starting the infusion. How would I bill for the s-code? Would there be a split in services and charges for these services w the home health company?
