attempted endometrial biospy, D&C,Hysterscopy and ECC

How do I code the following?

Doctor attempted endometrial biopsy on patient with post menopausal bleeding in outpatient surgical area. No anesthesia was administered per patient request. But doctor unable to dilate.One week later, paient was brought back to hospital and placed under general anesthesia. Doctor attempted D&C and Hysteroscopy but again, unable to dilate. Performed ECC and tissue was sent to pathology.( Results benign.)

How do I code for first attempt ( Endomterial Biopsy)?

How do I code for second attempt ( D&C,Hysteroscopy)?

How do I code for ECC done on same day (as attempted D&C,Hysteroscopy)?
