diagnosis code v03.2 billed with cpt 86580

i billed the cpt code 86580 with the diagnosis code v03.2 and mdcr dnd the claim as routine services not covered but even few accounts i have found we have received the payment so it creates a lot of confussion whether i should continue billing or not .Secondly, i need to know whether all the V codes are treated as routine check up.please clarify..............

ANSWER:

V03.2 (Need for prophylactic vaccination and inoculation against bacterial diseases - Tuberculosis [BCG]) is not the correct code to use with 86580 (Skin test; tuberculosis, intradermal). You are testing for the illness -- not vaccinating. If you are testing for tuberculosis, one of the appropriate V codes could be V74.1 (Special screening examination for bacterial and spirochetal diseases - Pulmonary tuberculosis) and/or any number of other V codes or symptom and condition ICD-9 codes which would most accurately reflect the reason the test was ordered. (i.e, cough, weight loss, HIV, malignancies, immunodeficiency, etc, etc).

As billed, it does imply prophylactic measure. The diagnosis code is not payable by Medicare for this service. No, all V codes are not considered routine.

how to identify which V codes are routine services and which is not....

Its really good to see the ASAP reply .How can we distinguish between V dx codes which is primary and which is not.
