LPN case managers

I am working in a family care clinic where I am an LPN case manager. I have been an LPN for 20 plus years and have been doing case management for a little over 1 year now. I do education for our DM population, obesity, hypertension patients. Are there any billable codes for these services? I have looked into the Medicare/ Medicaid guidelines but they are very vague. Any help with this would be greatly appreciated.
