History of colon polyps
If this is the reason for a return visit is it ok to use 211.3 instead of V12.72(hx of colon polyps.)

If they still have the polyp, I would use 211.3. If it was removed already and are just following up because of that, I would go with V12.72.

Yes. Medicare will not pay for V76.51.
