CPT® code 30905?

Hope somebody could help me
Patient was at the office for follow up Epistxis/Nasal Foreign body.

The DX code used 784.7 and 401.0.

And the CPT® code used 99212, 30905?

on epistaxis-- nares rocket removed. Hemostatis achieved

Was CPT® code30905 correct to apply in dx code 784.7.

I need some information from you guys,Hope that you guys can help me with this.. how can we bill for 784.7-epistaxis. The treatment done. Nares rocket removed. Hemostasis achieved. Patient tolerated it well. Can we apply 30905 for that dx code?
