Billing visits for patients in an assisted living facility

I have a physician who only sees patients in an assisted living facility. He bills 99334-99337 depending on the time he spends with the patients.

Can this code be billed more often than once in 30 days?

If this is a 30 day code will billing 99495 or 99496 effect the code at all and are both reimbursable in the same month.

I hope this is not too confusing, but I am a little confused.
