Methacholine Challenge test

In our practice (pulmonary) we do the Methacholine Challenge test (MCT) we were billing J7674, 94640, 94070, & 95070. This year 2012, we started getting denials for 94640 (as bundled) and they are only paying for either 94070 OR 95070. Can someone assist me in billing with the codes for billing this test please? I would really appreciate it.
