Routine vs. Medical Visit

Why are "blindness" diagnoses (H54.1 thru H54.7) considered Routine-related?

If patient comes in for a medical office visit due to/complains of blindness left eye, normal vision right eye- and thus billed primary diagnosis as such, why would medical insurance deny the office visit indicating "Routine refractive diagnosis" ?
