Coding Manuals

Unofficial poll. Which CPT and ICD manuals do you use and do you like them?

I teach at a local vocational school and we use AMA standard edition of CPT. I like it just becaue it has all the guidelines in it but it doesn't have anatomy or special references in it.
My personal copy I use at home is Ingenix professional edition and it has all the references to AMA CPT Assistant, some anatomy but NO guidelines.
I need a CPT that encompasses all of this.

At school we use Sanders/Carol Buck ICD-9 which is a bit overwhelming for the students because it contains so many notes, symbols and just looks kind of messy. What I do like about it is that it carries the official coding guidelines to the code section to help you code correctly. Ingenix doesn't do that but is neater, cleaner and more organized looking etc.

Any suggestions of publishers that contain all features I am looking for?
