observation to admit coding

I am trying to find out how to code the following scenario:

day 1-patient was placed in observation 99220—

day 2—dr decided to admit the patient. Full admit, nor more observation.

The decision to admit was not made on the same day as the observation. What series of CPT® codes would we use for day 2.

If the physician dictates an H&P, bill an initial hospital services. I would suggest a low level initial service. If the physician only writes a new, report a subsequent hospital visit.

Some payers may reject claims that have both obs and inpatient visits in the same stay. Some payers may prefer that you re code all of the physician services based on the status of the patient admission on discharge. That is, re code the initial and all subsequent visits based on the facility status. You have to check with the payer. Reporting each day based on the status of the patient that day would be correct coding but not all payer edits are the same.
