Filling out PEC forms

I have a question regarding similar DX.... If an Insurance company is asking if a patient was seen for lets say 250.03 during a certain time frame but they were only seen for 250.01 would you say yes and include the 250.01. I feel the two dx are different and would say no but this was sent by someone and they said yes and included that one dx that ins did not specifically ask for.
Any assistance in shedding light on this will help for future use and or correction of this last PEC form.
