We billed a pts insurance for a July of 2009 and 2010. The insurance has came back and taken the money back saying that it is a retroactive ineligiblity. We believe the pt has Medi-cal during those times of service. What is the correct modifier I need to use to bill the correct insurance? Or would a modifier be the correct way to rebill? Any advise would be appreciated. Thank You

How about billing the patient? You are likely past the timely filing period for Medi-cal and the patient did not properly inform you of her coverage so she is responsible for the costs. There is no modifier for "uninformed patient."
