1995 differences from 1997 guidelines

In the 1995 documentation guidelines for the HPI there is no mention of the "3 quiescent or chronic conditions________. I have never noticed this difference before. It was always my understanding the only difference between 1995 and 1997 guidelines was the exam. I just thought some of the other coders may not be aware of this little talked about difference.

Chronic diseases and 1995 guidelines

Many Medicare contractors allow the use of three chronic diseases using either the 1995 or 1997 Guidelines. CMS personnel have indicated this to people who have asked them, but never changed the guidelines (a lot of work) or indicated it in their manual. Look at your own Medicare contractor website for their E/M billing guide. Many of them have an audit tool and you can see for sure if/that your contractor allows it.
