Locum tenen npi

I have the guidelines on billing Locum Tenen docs, anyone know where on the claim the NPI's of the locum Tenen and the Physician he is covering for go?

this isn't required until 04/01/13 & CMS has not yet issued the instructions as to where this is to be reported on claims

So just the Q6 modifier under the regular doc they are covering for?
