Preventive Medicine visits vs. regular office visits

We are having increasing issues of a patient being seen for a follow up visit or some other issue and then they assume this was a "yearly physical" When they receive a bill, they argue that it was a physical and this is covered. What tips can I give our physicians to clearly document a regular visit vs. a preventive medicine visit (Codes 99381-99397) Thanks.
