So Confused

I need some coding clarification. First, with code 15273, when exactly is it used? If our doctors (plastic surgeons) apply Alloderm, Allomax or Flex HD what code are we to use? How about when they use Strattice mesh with hernia repairs and give me a cm size. Do I use 49568 even when they give me a size? Finally, is there a code difference for capsulectomy (which is what they usually do) vs capsulotomy (which they rarely do)?
