After the decision has been made to perform surgery, the patient comes in for another visit prior to the scheduled surgery to further discuss and answer questions related to the surgery and perform the surgical H&P. Can this visit be billed outside of the global professional fee associated with this surgery?

The answer is no, as you describe it. The pre-op visit is included in surgical fee package, even if performed outside "day-before" window.
