What is the best icd to use for this code

The best ICD is the one that is supported by the medical record. This is kind of a vague question. Per the CPT® description, it will probably be some type of skin infection.

Many years ago (really, back in the 80's,) I asked a biller in another practice why all the B-12 shots were being denied. She said, "you need to use dx code XXX" She should have said, "The covered dx is XXX and if your patient doesn't have that diagnosis, there is no medical necessity for the service."

I'm paraphrasing here.
