How do you file an office visit for a new patient, plus 20612, and J3301? Insurance says missing drug units and procedure code requires description.????Please help.

We would bill it as 20612 and J3301 x (10 mg units.) The drug units they are looking for is either mg or ml for the J3301. Our AHCCS plans have been requiring this for about 2 years.
